Terms and Conditions for Type 2 Diabetes Prevention Week Competition

  1. The promoter is NHS South Lincolnshire CCG whose registered office is at Bridge House, The Point, Lions Way, SLEAFORD, NG34 8GG.

  2. The competition is open to residents of the United Kingdom, aged 16 or over and registered with a GP practice who is a member of NHS Lincolnshire East CCG, NHS Lincolnshire West CCG, NHS South Lincolnshire CCG or NHS South West Lincolnshire CCG.

  3. The following individuals will not be eligible to enter (full details available on request). Members of the Lincolnshire Diabetes Prevention Steering Group, Executive Members of the four CCGs, individuals directly involved in Type 2 Diabetes Prevention Week events in Lincolnshire (and the immediate family members of these individuals).

  4. There is no entry fee and no purchase of goods is necessary to enter this competition.

  5. By entering this competition, an entrant is indicating his / her agreement to be bound by these terms and conditions.

  6. Entry requirements and routes of entry for the competition and details of how to enter are:
  • Make a pledge to support an improvement in your health and wellbeing (use a suggested example, or create your own). For email and postal entries, use the sentence “My pledge is…” Pledges may be detailed in promotional material, but will not include your personal details.

  • If possible, submit a photo detailing your intention, e.g. a photo of yourself holding a piece of paper detailing your pledge. If you submit a photo, it may be used in promotion material.

  • You can enter in person at one of our Type 2 Diabetes Prevention Week events:
    • Monday 1st April, 13:00 – 15:00 Grantham Hospital, in the area known as the ‘dog leg’ (near the stairs to the restaurant)
    • Tuesday 2nd April, 10:00 – 15:00, St Hildred’s Shopping Centre, Skegness
    • Thursday 4th April 10:00 – 15:00, Co-op, Seacroft Road, Mablethorpe

  • Your entry should include your full name and a contact telephone number or email address, to enable us to contact you if you are the winner.
  1. Only one entry will be accepted per person. Multiple entries from the same person will be disqualified.

  2. The closing date for entries will be 30th April 2019 (23:59 BST). After this date, no further entries to the competition will be permitted.

  3. No responsibility can be accepted for entries not received, whatever the reason.

  4. The promoter reserves the right to cancel or amend the competition and these terms and conditions without notice if circumstances arise which are outside of the promoter’s control.

  5. The prize is a FitBit Charge 3 with an additional option of a brief intervention session with a fully qualified health coach (ICS Health & Wellbeing). The prize is as stated and no cash alternative will be offered. The prizes are not transferable. Prizes are subject to availability and we reserve the right to substitute any prize with another of equivalent value without giving notice.

  6. The winner will be chosen at random by software, from all entries received, and verified by the promoter.

  7. The winner will be notified by email or telephone (as detailed) within 28 days of the closing date. If the winner cannot be contacted, or does not claim the prize within 28 days of notification, we reserve the right to withdraw the prize from the winner and randomly select a replacement winner.

  8. The promoter will make arrangements with the winner with regards when the prize can be issued. The winner will need to make themselves available for a promotional event to receive the prize (e.g. a meeting with the promoter and their communications team).

  9. The winner agrees to the use of his/her name and image in any publicity material, as well as their entry. The winner’s personal information will not be held for future use and will not be passed to any third parties without prior consent.  

  10. The promoter’s decision in respect of all matters connected with the competition will be final and no correspondence will be entered into.

  11. The competition and these terms and conditions will be governed by English law and any disputes will be subject exclusively to the jurisdiction of the courts of England.

  12. Personal data relating to entrants will be held securely by the promoter and will be deleted within 28 days of issue of the prize to the winner. Personal information will be held solely for the purposes of the prize draw and will not be passed to any third parties.

This form sends a non-secure free-text email to us. It has a similar security level as sending an unsecured email message from your email application (e.g. Outlook Express). Alternatively, contact us and we will send a form to you by post.
