South Lincolnshire Care Coordinators finalists at prestigious awards

One of the finalists at the recent Nursing Times Awards 2019 was a team of Care Coordinators from south Lincolnshire.

The Care Coordination Team, who are part of Allied Health South Lincolnshire, attended the prestigious awards having been shortlisted in the HRH The Prince of Wales Integrated Approaches to Care category.

Working across the south of the county, the team focuses on working with people and identifying “what matters to me”, moving away from “what is the matter with me” and so placing people at the centre of their own care.

“Our experienced and highly skilled team of Care Coordinators are nurses who work in GP practices, supporting patients who are frail and may be struggling to cope,” explains Kieran Harris, AHSL Operations Director.

“To best support the patient, the team leads a joined-up and collaborative approach, working with other care professionals in order to focus on the patient’s needs, with an emphasis on helping people to be fit and well, and remain in their community.”

Given the size and rurality of Lincolnshire, the team’s approach has made a significant difference to patients in the south who might otherwise have struggled to access services, but can now get the support they need when they need it.

“The team’s success was a key part of why it has was shortlisted in the Nursing Times Awards 2019,” adds Kieran.  “Whilst the team did not ultimately take home the award, accompanying them to the awards in London last week was quite possibly one of the proudest moments of my professional life.”

“For the team to be shortlisted is testament to the amazing work they do,” adds John Turner, Chief Officer, South Lincolnshire CCG.  “The work they, and Allied Health South Lincolnshire, do has not only made a huge difference to patients’ lives, it has also made a significant impact on reducing the strain on the health system as a whole.  I am proud of them and send them my warmest congratulations for their achievements, including the recognition from the Nursing Times Awards 2019.”
